O congelamento é uma resposta psicofisiológica do organismo, disparada quando o indivíduo se encontra em situações ameaçadoras e quando está em conflito sobre a melhor atitude a tomar (Alm, 2004a, 2005; Perkins, 1996). Na resposta de congelamento, há ativação simultânea do sistema nervoso simpático e do parassimpático. A ativação simpática induz ao aumento das funções (por exemplo, aumento dos batimentos cardíacos, aumento da sudorese e aumento da contração muscular), enquanto a ativação parassimpática induz à baixa de funções (por exemplo, diminuição da pressão arterial e diminuição da temperatura). A gagueira está codificada na &amp;quot;Classificação Internacional de Doenças - CID” (OMS, 2003a) com os caracteres F98.5, sendo considerada como distúrbio ou transtorno de fluência da fala. De forma complementar, a gagueira também está codificada na “Classificação Internacional de Funcionalidade, Incapacidade e Saúde - CIF” (OMS, 2003b) com os caracteres b3300, sendo considerada como uma deficiência da fluência da fala. &lt;br /&gt;
&lt;br /&gt;
A gagueira é caracterizada pela presença de repetições de parte de palavra (fone ou sílaba), alongamentos e bloqueios fônicos (OMS, 2003a) ou pela evitação ativa dessas hesitações (St. Louis &amp;amp; Myers, 1997). As hesitações gaguejadas ocorrem devido à dificuldade para iniciar o próximo fone ou a próxima sílaba da seqüência no tempo esperado. Desta forma, as hesitações gaguejadas são compensações da lentidão para iniciar o próximo segmento (Alm, 2004b, 2005). Por isso, diz-se que a gagueira é um distúrbio de temporalização da fala. Assim como ocorre com as hesitações comuns, as hesitações gaguejadas são maneiras de manter a fluência da fala e não de reduzi-la (O’Connell &amp;amp; Kowal, 2004). &lt;br /&gt;
&lt;br /&gt;
Acredita-se que tais dificuldades de temporalização ocorram devido à automatização insuficiente da fala pelo encéfalo (Alm, 2004b, 2005; Ludlow &amp;amp; Loucks, 2003). É vantajoso automatizar ações, porque o processamento tende a tornar-se rápido e pouco consciente. As estruturas encefálicas responsáveis pela automatização de tarefas são os núcleos da base, constituídos pelo agrupamento de cinco núcleos: caudado, putâmen, globo pálido (interno e externo), núcleo subtalâmico e substância negra. Estudos de neuroimagem evidenciam ativação anormal dos núcleos da base em sujeitos com gagueira, bem como ativação compensatória de regiões como cerebelo e hemisfério direito (Chang et al., 2008; Watkins et al., 2008). &lt;br /&gt;
&lt;br /&gt;
O fato de a gagueira ser um distúrbio de automatização da fala implica que o maior déficit está no processamento da modalidade mais automatizada de fala, ou seja, da fala espontânea (Alm, 2004b, 2005). Desta forma, há relativa preservação das outras modalidades de expressão oral (cantar, sussurrar, gritar, falar imitando outro dialeto, falar ou ler em uníssono, representar um personagem, falar com animais ou crianças pequenas). Esta constatação costuma intrigar leigos e pessoas que gaguejam, os quais freqüentemente concluem a favor de uma suposta etiologia emocional para o distúrbio, porque, segundo tal raciocínio, se a gagueira fosse uma alteração com base orgânica, a fluência deveria estar igualmente comprometida em todas as modalidades de expressão oral. &lt;br /&gt;
&lt;br /&gt;
Pesquisas científicas experimentais têm fortemente sugerido duas principais causas para a gagueira: hereditariedade e lesão encefálica (Alm, 2005; Drayna et al., 1999; Ludlow &amp;amp; Loucks, 2003). &lt;br /&gt;
&lt;br /&gt;
==Hereditariedade==&lt;br /&gt;
&lt;br /&gt;
Estima-se que 55% dos casos de gagueira sejam hereditários. A grande variabilidade fenotípica sugere que o tipo de herança relacionada à gagueira seja a oligogênica, ou seja, diversos genes atuando em conjunto para a geração do quadro (Felsenfeld, 1997). A proporção homem/mulher é de 1.6:1 (Drayna et al., 1999). Acredita-se que as mulheres necessitem de maior carga genética para gaguejar, porque conseguiriam compensar déficits de processamento lingüístico com maior facilidade, tendo em vista que a linguagem é mais bilateralizada no cérebro feminino em comparação com o masculino. &lt;br /&gt;
&lt;br /&gt;
Nos casos em que a gagueira é hereditária, geralmente há diversos membros na família que gaguejam de forma persistente ou que já gaguejaram em algum momento da vida (Felsenfeld, 1997; Yairi, 1997). Quando a gagueira é hereditária, ela geralmente inicia entre 2 e 4 anos, sendo disparada por um pico natural de dopamina que ocorre no cérebro. A dopamina é o principal neurotransmissor dos núcleos da base e está correlacionada com diversos comportamentos, tais como: automatização de ações, sensação de “pique”, grau de ativação motora, capacidade atencional e sensação de saciedade alimentar. Além disso, a presença de poucos traços de desatenção, hiperatividade e/ou impulsividade (TDAHI) também é um fator que aponta para a hereditariedade da gagueira, tendo em vista que o TDAHI está correlacionado com baixas concentrações de dopamina (Alm &amp;amp; Risberg, 2007). &lt;br /&gt;
&lt;br /&gt;
Nos casos hereditários, a gagueira costuma ser o único distúrbio de linguagem. É freqüente a aquisição precoce da linguagem. Acredita-se que um dos fatores responsáveis por esse fato sejam as altas concentrações de dopamina, que se correlacionam com altos graus de desempenho cognitivo (Alm, 2004b, 2005). &lt;br /&gt;
&lt;br /&gt;
Lesão encefálica &lt;br /&gt;
&lt;br /&gt;
Estima-se que 45% dos casos de gagueira sejam devido à lesão encefálica nos núcleos da base ou em regiões que se conectam a eles. Incidentes que sugerem a presença de lesão neurológica incluem (Alm, 2005): &lt;br /&gt;
&lt;br /&gt;
a) Hipóxia perinatal, geralmente ocasionada por complicações perinatais, prematuridade (menos de 8 meses de gestação) e baixo peso ao nascer (menos de 2,5 kg). Os neurônios encefálicos não são atingidos da mesma forma pela falta de oxigênio: os primeiros neurônios que sofrem são os do globo pálido, do cerebelo e do hipocampo (Descritores em Ciências da Saúde, 2008). &lt;br /&gt;
&lt;br /&gt;
b) Concussão, que é definida como um trauma leve de crânio, seguido de alteração transitória da consciência (Alm &amp;amp; Risberg, 2007; Segalowitz &amp;amp; Brown, 1991). As concussões são os incidentes neurológicos responsáveis pelos chamados “traumatismos ocultos”, ou seja, lesões encefálicas não detectáveis por exames convencionais de neuroimagem. Segundo Ashman et al. (2006), 28% das quedas, 20% dos acidentes de carro, 10% das batidas contra objetos e 3% dos acidentes de bicicletas geram traumatismos ocultos. Os grupos de maior risco para traumatismos ocultos são crianças entre 0 e 4 anos, adolescentes entre 15 e 19 anos e adultos com mais de 75 anos. &lt;br /&gt;
&lt;br /&gt;
c) Acidente vascular cerebral (AVC), caracterizado pela interrupção no fluxo sangüíneo cerebral devido à isquemia ou hemorragia. Os casos de gagueira pós-AVC são característicos de adultos (Theys et al., 2008). &lt;br /&gt;
&lt;br /&gt;
Nos casos de gagueira lesional, a gagueira inicia após o incidente neurológico, mas não necessariamente logo após. Pode haver um intervalo de meses ou até de anos entre o incidente e o início da gagueira. Além disso, costuma haver diversos traços de desatenção, hiperatividade e/ou impulsividade, que se constituem como outras manifestações da lesão neurológica (Alm &amp;amp; Risberg, 2007). Geralmente não há relato de outros casos de gagueira na família. &lt;br /&gt;
&lt;br /&gt;
A proporção homem/mulher na gagueira lesional é de 6:1 (Drayna et al., 1999). Sabe-se que os meninos são mais suscetíveis à hipóxia do que as meninas; acredita-se que a testosterona potencializa os efeitos da falta de oxigênio (Alm, 2005). Além disso, os homens apresentam maior probabilidade de se envolverem em situações de risco para lesão neurológica (Alm, 2005). &lt;br /&gt;
&lt;br /&gt;
Nos casos lesionais, geralmente há um maior comprometimento da linguagem, não sendo a gagueira o único distúrbio presente. É freqüente também haver transtorno fonológico, retardo de linguagem ou afasia (Theys et al., 2008). &lt;br /&gt;
&lt;br /&gt;

O conhecimento de senso comum sugere que a gagueira tanto é causada por fatores emocionais, quanto é fortemente influenciada por eles. Pesquisas experimentais indicam que apenas um fator emocional consistentemente influencia na intensidade da gagueira: a resposta de congelamento. O congelamento é uma resposta psicofisiológica do organismo, disparada quando o indivíduo se encontra em situações ameaçadoras e quando está em conflito sobre a melhor atitude a tomar (Alm, 2004a, 2005; Perkins, 1996). Na resposta de congelamento, há ativação simultânea do sistema nervoso simpático e do parassimpático. A ativação simpática induz ao aumento das funções (por exemplo, aumento dos batimentos cardíacos, aumento da sudorese e aumento da contração muscular), enquanto a ativação parassimpática induz à baixa de funções (por exemplo, diminuição da pressão arterial e diminuição da temperatura). A ativação concomitante de algumas funções que incitam o organismo à ação e de outras que o incitam à imobilidade gera, como conseqüência, a tendência ao congelamento (Alm, 2004a). &lt;br /&gt;
&lt;br /&gt;
Algumas emoções tendem a disparar a resposta de congelamento com maior facilidade. É o caso do medo, da insegurança e da vergonha. São emoções que tipicamente pioram a gagueira. Por outro lado, algumas emoções tendem a desativar a resposta de congelamento. É o caso da alegria, da confiança e da raiva. São emoções que tipicamente melhoram a gagueira. &lt;br /&gt;

Recuperação espontânea e cronificação &lt;br /&gt;
&lt;br /&gt;
Quando a gagueira inicia na infância, sabe-se que ocorre recuperação espontânea na maior parte dos casos. Três fatores têm sido apontados como os mais importantes para a cronificação da gagueira: presença de outros casos de gagueira na família (sugestivo de herança genética), presença de incidentes neurológicos (sugestivo de lesão encefálica) e criança do sexo masculino. A gravidade dos sintomas parece não se correlacionar com a recuperação espontânea (Saltuklaroglu &amp;amp; Kalinowski, 2005). &lt;br /&gt;

==Tratamentos==&lt;br /&gt;
&lt;br /&gt;
Atualmente há duas possibilidades de tratamento para a gagueira: fonoaudiológico e medicamentoso. &lt;br /&gt;
&lt;br /&gt;
O tratamento fonoaudiológico pauta-se basicamente na aprendizagem de estratégias que facilitam a fluência da fala (Giraud et al., 2008), na adaptação de aparelhos que manipulam o feedback auditivo (Stuart et al., 2006) e/ou na diminuição da resposta de congelamento (Alm, 2004a, 2005). Estratégias lingüísticas como a diminuição da taxa de articulação, o aumento no número e na duração das pausas silenciosas fluentes e o uso de hesitações voluntárias valem-se principalmente da manipulação do tempo de fala, fornecendo maior tempo para a liberação dos próximos segmentos fônicos. Por outro lado, a manipulação do feedback auditivo (retardo ou alteração de freqüência) vale-se da desautomatização da fala, tendo em vista que a pessoa que gagueja precisa sincronizar sua fala com um estímulo externo. Por fim, o trabalho com a resposta de congelamento visa à diminuição do grau de ameaça e de incerteza associados à gagueira. Pesquisas com neuroimagem funcional demonstram que o tratamento fonoaudiológico é efetivo na medida em que modifica os padrões de ativação encefálica para a fala (Giraud et al., 2008). &lt;br /&gt;
&lt;br /&gt;
O tratamento medicamentoso procura modificar as concentrações de neurotransmissores no encéfalo. Os inibidores seletivos de recaptura de serotonina (ISRS; como fluoxetina, sertralina e paroxetina) promovem um aumento na concentração de serotonina na fenda sináptica, devido ao bloqueio da recaptura da serotonina pelos neurônios pós-sinápticos. Os ISRS agem predominantemente sobre os sintomas ansiosos da resposta de congelamento (Kumar &amp;amp; Balan, 2007). Os neurolépticos (como risperidona, olanzapina e aripiprazol) bloqueiam parte dos receptores D2 de dopamina, diminuindo a entrada de dopamina nos neurônios pós-sinápticos. Os neurolépticos agem sobre a dificuldade de automatização da fala nos núcleos da base (Maguire et al., 2004). Todas as medicações podem produzir efeitos colaterais, devendo, sempre, serem prescritas por médicos. &lt;br /&gt;
